Referral Instructions



VISION USA STATE-RUN PROGRAMS

The following states participate in VISION USA using a local screening agency. Please refer to the instructions in your state.

ArizonaTo be screened in Arizona, please call 800/346-2020.

California

To be screened in California, contact the California Vision Foundation:

ColoradoColorado is revising its program this year and there will be no examinations until further notice.

HawaiiTo be screened in Hawaii, contact Hawaii Optometric Association, 220 S. King St., #801, Honolulu, HI 96813-4526, or call 808/537-5678. Ask about VISION USA.

KansasTo be screened in Kansas, contact local Salvation Army or call 816/756-1455 and ask for Service Extension Dept., or write Salvation Army, 101 W. Linwood, P.O. Box 412577, Kansas City, MO 64141-2577.

KentuckyTo be screened in Kentucky, write for an application, Kentucky Vision Project, P.O. Box 1422, Frankfort, KY 40602 or go to www.kyeyes.org for an application or any Salvation Army.

Minnesota
To be screened in Minnesota, contact the Salvation Army, 401 W. 7th St., St. Paul, MN 55102 and ask about the Minnesota Vision Project or call 651-746-3400.

MontanaMontana is not participating but you can contact the Montana Optometric Association, 36 S. Last Change Gulch, Ste. A, Helena, MT 59601 or call 406/443-1160 for referrals to other groups that might be able to assist you with a free eye exam.

North DakotaTo be screened in North Dakota, write or call for an application. North Dakota Optometric Association, 921 S. 9th St., Suite 120, Bismarck, ND 58504, 701/258-6766.

WisconsinThe Wisconsin program is limited to children up to 18. To be screened in Wisconsin, write VISION USA, the Wisconsin Project, 6510 Grand Teton Plaza, Suite 312, Madison, WI 53719, 877/435-2020, or fax: 608/824-2205.

WyomingTo be screened in Wyoming contact the Wyoming Dept. of Family Services in your county. Look under "W" in your telephone directory or call Dept. of Family Services, 307/777-6800. Ask about "Give One Day" program.
